WARD VISITS

                         
Objectives:
                      The main objectives of ward visits
1)      To observe the case taking and case handling
2)      To see the role of Pharmacist in establishing of Pharmaceutical care plans
3)      Implementation of Pharmaceutical care plan
4)      Noting any discrepancies in prescriptions and medication orders.
5)      Methods of Administration to Patients
6)      Finding any possible Drug-Drug Interaction
7)      Stock management and record keeping in satellite Pharmacies
8)      To observe the medication available at nursing stations and in emergency tray.
Problems:
1)      In most of wards the space for Satellite Pharmacy was not adequate
2)      The arrangement of medication was not proper
3)      Due to poor arrangement FEFO or FIFO principle was difficult to implement.
4)      Record was not maintained
5)      There is a swear shortage of   Staff.
6)      The storage conditions in the Satellite pharmacy was very poor
7)      Date was not mentioned on pre-filled syringes
8)      Refrigerator items were not stored properly.
9)      Disposable material like used vials and ampoules were not discarded properly and were seen at nursing stations.
10)  There was no involvement of Pharmacist in the wards.
Suggestions:
1)      The no of dispensing staff should be raised
2)      SOP’s should be implemented.
3)      A Pharmacist should be at every satellite Pharmacy
4)      If (3) is not possible then Hospital should adopt the Unit Dose Dispensing system to avoid the Pilferage of Medications.
